Advance directives

If you have a Living Will, please bring a copy with you to every hospital inpatient and outpatient visit. If you do not have a Living Will but would like one, please speak with the Admitting Office upon registration.

Outpatient testing

Make sure your doctor has called the hospital to order your procedure. Or, if the doctor gives the order to you, be sure to bring it with you when you report for testing.

We are committed to maintaining the privacy and confidentiality of all patient records. If you need a copy of your medical record, please call (813) 634-0167. We are pleased to serve your medical record requests.

Privacy password

At admission, every patient is given a confidential password. Patients are told to share it with people they want to receive their confidential patient health information. Nurses and staff can provide information about the patient’s health ONLY to people who know the password. This password is only for verbal communication regarding treatment or payment during this hospital stay. It will not be honored once the patient is discharged.

Valuables

Leave unnecessary valuables at home. Be prepared to pay your co-payments and deductibles, but do not bring large amounts of cash with you. We cannot be responsible for the loss or breakage of personal items brought into the hospital.

Accounting of Disclosures policy

HCA Florida South Shore Hospital provides an Accounting of Disclosures policy as approved by our Ethics and Compliance Policy Committee.

The purpose of our Accounting of Disclosures policy is to ensure that each Company-affiliated facility, and their respective departments, understands the requirement to populate and provide an Accounting of Disclosures of Protected Health Information to all patients as required by the Health Insurance Portability and Accountability Act (HIPAA), Standards for Privacy of Individually Identifiable Health Information (Privacy Standards), 45 CFR Parts 160 and 164, the Health Information Technology for Economic and Clinical Health Act (HITECH) component of the American Recovery and Reinvestment Act of 2009 (ARRA), and any and all other Federal regulations and interpretive guidelines promulgated thereunder.

Company-affiliated facilities include, but are not limited to, hospitals, ambulatory surgery centers, imaging and oncology centers, physician practices, shared services centers and each entities' respective departments.
