Advance Directives

If you have a Living Will, please bring a copy with you to every hospital inpatient and outpatient visit. If you do not have a Living Will, but would like one, please speak with the Admitting Office upon registration.

Complaints

South Bay Hospital wants to be your hospital of choice. If you have questions or concerns during your hospital stay, please call our Administrative Department at (813) 634-0106.

Insurance Information

Please bring your insurance card when you are admitted. We will make a copy of your card so we can bill your insurer directly for your covered benefits. Your co-payments are expected at this time. It is important that you understand what your insurance covers and where you can go for care. Many insurance plans require pre-certification before admission. To avoid reduced benefits, call your insurance company to verify your coverage and make sure your authorizations are in order. If you have any financial questions, please call our Financial Counselors at (813) 634-0173 or (813) 634-3301 x1033, weekdays from 8:30am – 5:00pm.

Outpatient Testing

Make sure your doctor has called the hospital to order your procedure. Or, if the doctor gives the order to you, be sure to bring it with you when you report for testing.

Medical Records

We are committed to maintaining the privacy and confidentiality of all patient records. If you need a copy of your medical record, please call (813) 634-0167. We are pleased to serve your medical record requests.

Medication

Keep track of your prescriptions and set reminders to take medications and order refills by going to www.MyMedSchedule.com.

Privacy Password

At admission, every patient is given a confidential password. Patients are told to share it with people they want to receive their confidential patient health information. Nurses and staff can provide information about the patient’s health ONLY to persons who know the password. This password is only for verbal communication regarding treatment or payment during this hospital stay. It will not be honored once the patient is discharged.

Valuables

Leave unnecessary valuables at home. Be prepared to pay your co-payments and deductibles, but do not bring large amounts of cash with you. We cannot be responsible for loss or breakage of personal items brought into the hospital.
